[HSF] RV failure management strategies

Actually the preop RV function was pretty bad preop but I thought 
correction of the left sided lesion and correcting the TR may make 
things better. Unfortunately the PA pressure continued to be pretty high 
despite low wedge and that  seems to add to the RV  problem. I did 
religiously give cardioplegia though whether that constituted 
"protection" may be debatable !! :-)
