For me I would replace the aortic valve with a mechanical valve and repaced the asc aorta with a short tube graft. Would confirm that there was no residual PDA or coart and probably explore the main PA and repair with verticle arterioplasty or very short tube graft possibly improving the PI. If the PAs are small and no PAH I might do nothing to the PA. Now that I have played my cards, I would like to hear from the pedi guys also.

Subject: [HSF] PA Aneurysm
A 39 yo normal appearing and healthy female presented to her primary care physician with chest pain. He ordered a 64-slice CT scan which showed a 4.6cm main pulmonary artery aneurysm and a 5.2 cm ascending aortic aneurysm originating at the root and tapering to normal size at the inominate artery.
A TEE demonstrated a bicuspid aortic valve with severe aortic insufficiency, trivial pulmonary insufficiency, and a slightly depressed left ventricle. Angiogram showed normal coronaries.
What type of operation would you perform?
Has anyone encountered this situation in an adult where both aorta and pulmonary artery need to be addressed?
Looking forward to comments:
