I personally use the pump around 5% of the time occasionally more depending on the mix, but I rarely use the cross clamp for isolated CABGs. I think less than once a year. I did say off pump not beating heart which is the distinction I think that you are making. I don't really like the rigid "off pump" classification vs "on".

Hal.....there IS consensus mitral valve repair is superior to
replacement but NOT everyone is doing it.....Although no consensus re:
superiority of OPCAB evidence continues to mount......OPCAB IS more
difficult to perform requiring a different "mindset"and
experience,perserverance,and patience to overcome learning curve(ala
mitral repair)........bill turner

  Whew!  Take a breath!  All I can tell you is that there is  no
consensus in 
the surgical community that OPCAB is superior.  Otherwise,  everybody
would 
be doing it.  I happen to like it for the right p
atient,  because I think the 
pump is inherently bad for the brain.  However, you  have to admit that
the 
evidence is not overwhelming.  They've been debating  this for years at
the 
national meetings.
