I wonder if Dr. Salerno's question was rhetoric. I can not believe a surgeon
of his caliber would be so ignorant and out of touch of American CABG
surgery. Most well known institutions in the US, including the Mayo Clinic
and the Cleveland clinic do not do much off pump surgery at all.

PS Incidentally, I think this Forum's great strength is the practical,
clinical advice you get from the many skilled, talented and experienced
surgeons (like Dr. Salerno, for example). But we do a poor job when we
venture into sweeping generalizations based on our individual, unscientific
biases, not supported by evidence.
Fortunately, I am sure, most of us can separate the wheat form chaff.

Sure, most of the cases I do are on pump. As far as I know nearly 80% of the
cases and surgeons in this country still use the proven technique that is
easily reproducible.

Hal.....there IS consensus mitral valve repair is superior to
replacement but NOT everyone is doing it.....Although no consensus re:
superiority of OPCAB evidence continues to mount......OPCAB IS more
difficult to perform requiring a different "mindset"and
experience,perserverance,and patience to overcome learning curve(ala

Whew! Take a breath! All I can tell you is that there is no
consensus in
the surgical community that OPCAB is superior. Otherwise, everybody
would
be doing it. I happen to like it for the right p
atient, because I think the
pump is inherently bad for the brain. However, you have to admit that
the
evidence is not overwhelming. They've been debating this for years at
the
national meetings.
