[HSF] non cardiac surgery in pts with metal valves

same policy. I stop Coumadin and start heparin "PTT double Normal Control"
.... 3 days before surgery, till INR is normal  ... stop Heparin 3 hours
before surgery, to commence it 3 hours after surgery. re-institute Coumadin
again, over 3 days .... to withdraw Hep then.
