Well, you create it.  :)  
- You promote the floozies by finagling choice assignments for them...or
making your big adorers the "resource nurse" -- thereby cutting them loose
to spend time at your feet sucking up to you.  

- You select your friends for unique jobs during a shift. And then, when
unique opportunities arise, only your friends are eligible.  

- Oh, also coordinate your schedules, so you always work with your friends.

- If new nurses get hired who resist being assimilated, you assign them to
the back forty where they can feed, water, and turn the patients to the
sun.....while you and your friends enjoy an interesting shift with the
exciting work.  

- Pretty soon you are the leader of a fiefdom of groupies.  :)
