Ben,
  I'm pretty simple minded, but if heavily bleeding patients require  blood, 
they are going to require a lot of blood transfusions.  They are  also more 
likely to be injured from sustained hypoperfusion.  They are  going to be more 
likely to die from multisystem organ failure (lungs, liver,  kidneys).  The 
increases mortality and the increases transfusion  requirement may be true, true/ 
unrelated.  It doesn't matter if the  bleeding was during a CABG gone bad or 
an acute dissection.
