Tom,
  I don't know how many units of blood you normally use on a type I  
dissection, but I wouldn't operate until you think you have  enough.  As we all know, 
it can take the blood bank quite a while to  work through the crossmatching 
problems associated with preformed antibodies.  With that frightening history of 
GI bleeding, it may be that his best option is  medical management.  Good 
luck.  BTW, I would use aprotinin on this  case!
