I have sent the image of the HPE of the perimount valve which Roberto 
sent me.

here is the histology of the perimount calcified valve. Only calcifications.
No signals of subtle endocarditis as Guilo Rizzoli proposed to look for.

Die  Patientin heißt Regina Böttcher, geb 30.11.1936

Leider haben wir das Mail vom Bob Frater aus USA am 09.10 bekommen, Sie ist
am 04.10 operiert worden.
In den Einsendenschein aber wir haben einen Wunsch vom Photo geschrieben.
Trotzdem, freue ich mich auf ein  Mikroskopisches Photo.
Alle nächste früh-explantierten Bioklappen sollten wie unten vom Bob Frater
geschrieben untersucht werden. Normalerweise wir versprechen aller Patienten
15 Jahre Garantie...

da die Klappenprothese bei uns am nächsten Tag eingeht und zugeschnitten
wird und am übernächsten Tag bereits der schriftliche Befund erstellt wird,
erreicht mich Ihre Nachricht sowohl hinsichtlich des makroskopischen Photos
als auch einer Kalkmaßangabe. Gerne bin ich bereit Ihnen ein mikroskopisches
Photo zu senden, vorausgesetzt Sie nennen mir Name und Geburtsdatum des
Patienten.
Gerne wäre ich Ihnen weiter entgegen gekommen, es wäre günstig, wenn Sie
Ihre Wünsche hinsichtlich der Makroskopie bereits auf dem Einsendeschein
vermerken würden, so dass der Zuschnitt bereits speziell darauf ausgerichtet
werden kann, andernfalls erfolgt die Standardbearbeitung.

Bitte schreiben Sie mir, was für Sie jetzt noch von Interesse ist.
Vielleicht kann man sich auch noch einmal am Mittwoch zur K-Konferenz kurz
darüber unterhalten.

Sehr Geehrte Tanja,

Ich habe vor 5 Tage eine Perimount Bioklappe explantiert, nach 3 Jahre
Implantation. Die Klappe müsste untersucht werden wenn möglich wie unser
Kollege Robert Frater aus USA uns racommandiert hat(siehe Mail).Es ist sehr
selten dass eine Bioklappe mit antikalzifikation Behandlung, so früh
verkalkt wird.
Wir bedanken uns auch wegen eine gute Photo, wir haben eine gemacht aber ist
außer Focus.

Dear Roberto,
First let me say that you removed the valve the way it should be removed. 
During the FDA trial for the Perimount in the aortic position (I was senior

author on the first publication), we did not see early calcification. 
The calcium is in all three leaflets. It is in one leaflet in large  amounts

at a high stress area but surprisingly, is not obvious in the major  high 
stress areas which are the commissures.
Possibilities to be thought of:
1) Patient. Hyperparathyroidism. Renal failure. Other abnormal calcium  
metabolism. Calcium affecting medications etc.
2) Pericardium. In all rat subcutaneous tests for material  calcification 
there are outliers that show more calcium than the other samples.  This is 
possibly related to donor differences and, less likely, to post  harvesting 
departures from the normal treatment of a batch  of pericardium. From the
serial # the 
company may be able  to see if there are other cases from the same batch.
3) Fabrication of the valve. Handmade valves have the potential for being  
put together differently. This can alter the stress in a leaflet. This is  
unlikely and the valve has no obvious irregularities that I can see. 
4) Secondary valve disease with subsequent calcification. e.g. infection  
with a calcified vegetation, or thrombus formation also with secondary  
calcification. 
The valve should receive a careful histological analysis form a pathologist

with an interest in bioprostheses. I would also take a strip from one of the

other two leaflets from edge to base and get tissue calcium in mg/G dry 
weight.  The difficulty will be in finding the expected norm for pericardium
after 
three  years of  human implantation.
Roberto it is good that you are devoting thought to this. So often a   valve

like this is, believe it or not, discarded,
