[HSF] [OT] Carpentier's Heart Valve Club Meeting

Hal, I do leave 2-3 mm of the margin at times unclosed as it allows an
"axis of closure" of the "pinafore" while the valve is closing. Closing
right up to the edge at times can prevent a curvilinear closure.

Dreyfuss did a nice repair with papillary muscle repositioning. Dr. Dion was having quite a bit of trouble with a redo valve repair. It looked like to me he should have closed an interscallop leak at P2-P3 or done a leaflet extension. I'll bet he ended up with a replacement. I hate to say it, but I kind of got a little grim satisfaction knowing that the big shots also struggle.
> Like you, I thought the Heart Valve Club meeting at Sinai was one of the best 2 or 3 valve meetings that I've ever attended. I was a little disappointed that Carpentier turned out to be human (he had to take one repair back the following day). In my mind, David Adams has really matured into one of the top minds in valve repair today. His systematic, innovative, undogmatic (is that a word?) approach to valve repair is one to be followed if one is going to get his repair rate up into the 90's. Using methylene blue to paint the line of closure so that you can tell the depth of closure is a neat trick. He also advocated leaving the terminal 2-3 mm at the leaflet edge open when closing an interscallop leak so that it wouldn't be retricted at the edge. I plan to try that. Using vertical compression sutures along with horizontal compression sutures when doing sliding leaflet plasty looks like it would further releave any tension on the suture lines.
> Faisal, it was great meeting you as well. You were one of several guys who introduced themselves to me. I'm glad that at least a few of you appreciate the input.
