We did not have a pyramid type program and except for letting me thru it seemed to work pretty well. Fear is not a particularly good teacher, even if it may focus the mind. Neuroscience might suggest that focusing the mind is counter productive. (My words/ concepts are a little loose.) Does having a lawyer look over your shoulder help you do better, too?

Basically, all that does is make it so that the future CT surgeon merely
goes through the paces of the general surgery residency. He completes his 5
years and moves on to cardiothoracic residency no matter what. What a farce!
I can't tell you the untold hours I spent reading Schwartz, journals, and
whatever I could get my hands on as a general surgery resident, knowing that I
had to excel on the In Training Exam if I was going to beat the pyramid. We
went from 10 interns to 4 chiefs at the Univ of Maryland and I was the only
one from out of state who survived the cut. Let me tell you, those first
couple of years were scary as heck, because I new that I could easily end up as an
anesthesiologist or ER doc.
