I did quite a few mitral procedures via the aorta and some aortic maneuvers through the mitral orifice (not as good). It so happens I don't think that I did exactly what you describe. But so what if I or anyone in the world has never done it before. Congratulations on a good solution!
By the rules I once worked under I was theoretically required to get approval for any procedure that had not been done in the hospital before. I absolutely ignored those rules through nearly half a century, Which does not mean that I did not getall thehelp I could from trusted colleagues anywhere in the world

I just finished a very difficult redo avr, mvr (both replacements). With the
aortic valve out, I still had trouble seeing the anterior part of the annulus,
even by usual technique of progressively putting annular sutures and "sneaking
up" on this part of the annulus by pulling down on the previously placed
sutures. I finally had to place the last three pledgeted matress sututres via
the aortotomy and pass them into the left atrium for placement through the valve
ring. It worked remarkably well. Has anybody had experience doing the
difficultly exposed anterior annulus this way?
