[HSF] Bicuspid aortic valve and dilated ascending aorta

Thanks, Hal. I agree. Diagnostics aren't a problem. But in the absence of 
AI, increased LVEDD, etc., I'd like a little consensus  on when to 
recommend surgery. 4.5cm? 5?
