Justin,
  There's nothing wrong with deploying an IVC filter from the IJ in  this 
situation. As to whether or not to operate, that's a tough call.  My  experience 
with the Trendelenberg procedure in these cases is not very  positive.  As 
long as she doesn't have hemodynamic compromise, I would vote  for heparin and an 
IVC filter as the first line of therapy.
