How big was the hernia operation?
Keep her controlled, may be you need to operate...
Keep us informed. We have the trend to operate earlier than after
decompensation, based on CT´s, but again, this is not evidence based.

Justin,
There's nothing wrong with deploying an IVC filter from the IJ in this
situation. As to whether or not to operate, that's a tough call. My
experience
with the Trendelenberg procedure in these cases is not very positive. As
long as she doesn't have hemodynamic compromise, I would vote for heparin
and an
IVC filter as the first line of therapy.
