When was in KEM we used to get a lot of Aortoarteritis cases and some
had occlusion of 3 of four cerebral vessels occluded and ungraftable
with the remaining one being stenotic and we used to do an aorto
carotids for them with deep hypothermia as a cerebral protectant. I
still vividly remember one extreme case - she was a girl who was brought
to us who would get syncope and convulse if she was propped upright. She
did well with this strategy. We did expose the opposite carotid (the
occluded one) which was totally cord like. We had planned to first put
it to the occluded artery first (to establish some antegrade flow) if we
get some lumen before going on to the non occluded side.
