Hal,
I didn't mean to negate the fact that platelet concentrates are neccessary in Bentall but just to show our very different world than yours.As I previously have told you we don't have a blood bank here.Just a station to procure blood from donors and convert it to rbc concentrates and plasma.( several averagely qualified technicians) Also we don't have any hematologist or any internist with blood bank expertise.this is  a rural area and most of the patients don't have the neccessary means to travel to other cardiac centers .Also as I previously had mentioned we don't have a nephrologist and my team do RRT if a neccessity arises.
Follow up on the patient:Hb 10.1 Htc:29,Platelets :87000 Total Bilirubin:28 ( down from 37 12 hours ago) ,Cre:2.1 ( down from 3.8 24 hours ago).He is alert and stable.
