You guys are smart enough to prove that I am an amateur for carotid surgery.
In my area, neurosurgeons take care of carotid disease. In past few years, I
experienced only one case who had bilateral significant carotid stenosis and
TIA and worsening angina with 3VD. In that case, prior to OPCAB, bilateral
stent implantation were performed without neuro events.

I am certain that the prevalence of carotid disease is much higher in
western countries, and my friend neurosurgeon also told me the anatomy of
carotid bifurcation in Japanese is located (one vertebra) higher than that
of westerners.

So let me ask you about how many simultaneous TIA and unstable AP requiring
CABG+CEA/stent you encounter per year. What is differential indication of
carotid stenting/endarterectomy currently in US (or your practice)?
