  What that tell's me is that certain surgeons probably shouldn't be doing carotids.  I would bet you that the vast majority of experienced carotid surgeons would have better neurologic outcomes than the 6-8% complication rate I've seen with most carotid stent series.

n an European randomized study the stroke % was the same with stent or
urgery, BUT the % difference  of complications between surgeons was greater
han between both methods.

I agree with Roberto about using combined CEA/CABG instead of  carotid 
tenting followed by CABG.  Is there any data to suggest that this  latter 
pproach results in fewer strokes?  If you don't routinely do CEA, I  can
nderstand 
hy you'd want to get the carotid stented first, but is it truly  the safer 
pproach?  From everything I've read, the stroke rate for stented  carotids,
even with the latest generation of protection devices, is consistently
igher 
han CEA.
