Unless it really is severe nothing needs to be done differently. I have never done it, but if the heart was severely displaced by the pectus, I suppose it would be easy enough to take out the sternum removing the most of the cartilage,  divide the manbrium,  and put back in the sternum over the fascia after the CABG.
