I have (call it serendipity) got a child with Pectus who requires an MV
repair!!
We had done a few cases with variable amounts of pectus but left them
alone. One had a pretty severe pectus but the patient ws not interested
in repair. Shje was scheduled for an OPCAB but when we opened cardiac
dislocation was difficult and was done on CPB.
I would consider that if you needed to do a pectus repair IMA's would be
kept preserved for a Wada flipover.

I have never done it, but if the heart was severely displaced by the pectus, I suppose it would be easy enough to take out the sternum removing the most of the cartilage, divide the manbrium, and put back in the sternum over the fascia after the CABG.
