Is the mechanism for the RV infarct not dissection down the coronary sinus? If so what is to say the left sinus will not dissect next infarcting the rest of the heart?
Hal I think you have to be very clear as many students, residents and novice surgeons as myself read this forum regard comments from senior HSF surgeons as yourself very highly. Are you seriously saying that when a dissection presents as hemopericardium and myocardial infarction with RV failure we should consider a strategy of CABG only?
