Many "established surgical principles" were never based on clear rationale 
and logic. I venture to say that many surgeons practice by established 
principles and have no idea what rationale and logic led to their establishment. I 
often ask surgeons younger than myself (which includes most of them) why they did 
something in a certain way and the usual reply is either that it is the way 
they have always done it or the way that they were taught. Rarely is the 
question "Why?" addressed. John Flege
