Many "established surgical principles" were never based on clear rationale
and logic. I venture to say that many surgeons practice by established
principles and have no idea what rationale and logic led to their
establishment. I
often ask surgeons younger than myself (which includes most of them) why
they did
something in a certain way and the usual reply is either that it is the way
they have always done it or the way that they were taught. Rarely is the
question "Why?" addressed. John Flege
Nor John do they reply "I tried the X method for 12 cases and in my hands I
had 3/12 poor results so I tried method Y for 10 cases and thought the
results were better. To be certain I randomised my next 20 cases and sure enough
method Y was better. SInce then I have been using method Y but last year seeing
what A has been doing and recognising that his method makes things easier
have modified my technique and am watching the results carefully to be sure
there are no surprises.
I have often quoted Clagett what said to Memories on HSF are short so I
shall repeat myself): "While you are with me do it my way. If you are still
doing it my way in 10 years time I shall be very disappointed in you".
