I have voiced that same concerns in earlier postings. Adding "stuff" PVI, CPB, ligation LAA,etc to the shopping cart is never free, even if the security guard does not stop you.

PVI still scares me for a variety of reasons - esp in the elderly (I saw
some tear a hole in the posterior PV and it was a bitch to fix), but my
concern is I have seen a few patients, such as yours, who get "uncomplicated
operations" (of all ages) who look like crap and can never figure out why
and the only common denominator is the PVI.  I always have Anesthesia make
sure there is flow thru all the PV at the end to make sure I did not fused
them closed (I am not sure if that is possible), but I am concerned that PVI
(like LAA ligation) is not entirely benign.

Did your patient have a PE?
