Certainly when and if you find yourself correcting the mitral from the ventricular side for the first time, you do not want to draw any lessons from a surgeon who has done a thousand operations through the ventricular approach.

I've never had to replace a mitral valve while doing a ventricular
aneurysm. If and when I do, I'll try to do it from the ventricular side. One thing
I'll never do, however, is a ventricular side DeVega as recommended by
Menicante. When doing a mitral repair for annular dilatation, a rigid or
semirigid ring is necessary for a durable result.
