Dear Tohru, I would like to comment on :Therefore I stay away from routine> y-composite and/or endarterectomy as much as possible.1.In our practice with so many short and obese ladies with very poor quality veins and short aortas comparable to their height ,we are obliged to do many vein on top of the vein proximal anastomosis.
2.With a very high percentage of diffuse CAD in a fair percentage of the population we are forced to do localized or extended endarterectomy to even LAD ( much more common to RCA) or make the distal anastomosis to the distal portions of the coronaries. This raises an unanswered question for me : how far can or should we construct the distal anastomosis in the coronary system.
