Dear Theofilo,
  Again, my point is that surgeons who are 100% OPCAB'ers probably  
occasionally deprive their patient of a well done endarterectomy (note, I said  well 
done).  That would be like me repairing 100% of the mitrals I  encounter.  With 
heavily calcified rheumatic valves, the repair would be  substandard and 
better served by replacement.  Surgeons need to be able to  pick and choose the 
best technique for that particular patient.  Beware of  anyone who is an 
extremist, whether religious or surgical.
