I think that traumatic transsection and young ductal aneurysm may be ideal
for stent graft. However most aortic arch aeurysms ( or distal arch
aneurysms ) should be treated with conventional approach. Actually we have
done many of them with reasonable risks. With our exposure technique, we can
make distal aortic anastomosis as deep as the level of left hilum. These
aneurysms, atheroscrelotic, are so-called "tip of iceberg". Cheese-like soft
atheroma is all over! Compared to "good" open technique, blind introduction
of stent graft have tremendous morbidities and mortalities. I would not give
a shit.

I put my recent case 3DCT images as attachments (preop and postop, I hope
server accept them!) 78 yo male with 3VD, atrial fibrillation, and distal
arch aneurysm. I operated on him triple cabg, RF maze and total arch
replacement with branched woven Gelweave. This kind of operation is one of
our routine cases. Is this kind of procedure common in US or other western
countries?
