Erdinc,
  With a 5 mm vegetation, I guess I would wait.  Close  observation, serial 
blood cultures, WBC's, etc.  The TEE should be repeated  in a week.  If you 
have to operate, I recommend AVR, mitral and tricuspid  repairs.
