I agree with Roberto. I would operate sooner rather than later.
Three weeks back, I was referred a 52 yr old man with acute AR with pulm
edema. He was on dopamine and frusemide infusions. Creatinine 200. Blood
culture strep viridans. There was a 1.2cm mobile vegetation on the left
coronary cusp (picture attached). At operation, in addition there were three
perforations on the right coronary cusp. The patient wanted a bioprosthesis.
We were planning to filter him on bypass, but he passed 800ml of urine. We
loaded him with milrinone before coming off bypass.He recovered well in ICU.
Bak to ward day1. Had bilateral pleural effusions which were aspirated. Went
home day7 with normal creatinine.
