  I don't begin to know the fee structure for Google's profits, but I  can 
tell you it's definitely not 100% from advertising.  I understand  people and 
companies pay some sort of fee to make their name towards the front  of the 
list.  For example, Google "mitral repair".  Mt. Sinai is at  the head of the 
line.  Are they the uncontested top spot in the world for  mitral repair?  I think 
they are great, but I can think of several at least  comparable centers.  I 
would gladly bet you that they pay some significant  amount to get that 
position.
