One thing to remember Prasanna is that many critical contributions appeared  
first in books rather than journals.One example is ventricular - aortic valved 
 conduits for aortic stenosis  which is in the Proceedings of the 1955 Henry  
Ford Symposium on Cardiac Surgery.
Joseph Skoda's definition of the different function of First and Second  
order chordae is in a book on Perkussion und Auskultation published in Vienna in  
1850.
Other questions: Do you cite the first reference to an idea or  concept or a 
reference to its significant translation into general clinical  practice? 
Whose list  do you follow? Reaching consensus could be a great  exercise but would 
take an enormous amount of time; that is why we have  historians who work 
steadily for fiv years to produce one book.
What you are doing is very importantfor your trainees. Keep up the good  work!
