Usually I do endartorectomy in those vessels totally occluted, and just use the blower to feciliated the anastomosis for the incision in the CA usually pretty long the the shunt is not longer enough. I do both LAD and RCA endartorectomy. If you can finish the anastomosis within 6 to10 mins, I have not seen any problem. 

We give LMH for the first 24 to 48 hours and then ASP and PLevix for at least 6 month.
