The "lumen" is the right atrial chamber.  Ordinarily, I would agree  with 
merely anticoagulating him, but this clot is attached throughout the right  
atrial wall and occupies 2/3 of the chamber.  I suspect at operation it  would be 
highly reminiscent of an AAA in which the "lumen" is nearly of normal  size, 
but there is lots of laminated clot in the wall of the aneurysm.
