Dear Claudia,
  Thanks for the info.  Please send me the additional paper if  you would.  I 
don't know the answer to your questions as yet.  
  The latest on the patient is that he checked himself out to have  this 
worked up as an outpatient.  I think this guy is more than a little  crazy.  He is 
a construction foreman and seems to be in a bit of  denial.  Anyway,  he 
supposedly will followup in my office.  I'll  keep you posted.
