This is a picture of a common association of rheumatic MR - PML 
retraction and anterior leaflet prolapse.
Minimal commissural fusion is release by commissurotomy (essential to 
avoid a hinge point problem).Release of the PML by release of 
posterobasal chordae is critical and the AML is shortened by combined 
chordoplasty. The whole thing is guarded by a ring.
