If at the center of an atrioventriculanterior cleft the free edges veer away from each other doing a complete closure of the cleft ma straighten the free edge of the aanterior leaflet and leavesignificant insufficiency. A small triangular notch in the anterior leafletn left by not stitching the whole cleft leaves very little leak.

Yes. The only place where we need to completely close upto the edge (and even there rarely it may be necessary to avoid it (ie complete cleft closure) in case stenosis will result) is in a primum ASD. Of course the mechanism and pathology are different.
