Also if it just cannot be repaired due to extensive excision you can 
patch of that commissure something like an Alfieri and actually shifting 
the neoannulus more medially.
If you see the survival curves for repair and replacement , repair beats 
replacement and active versus healed helaed beats active (obviously)
