Hal, Prasanna said that patients with repaired mitral valves come back 
if they have a problem. Presumably they  then have another operation in 
contradistinction to patients with failed mechanical valves. Patients 
with failed biologic valves usually survive to have another operation. 
Why would you put a mechanical valve in your 40 yr old patient when you 
are worried about catastrophic failure? John Flege

   Great point.  If I hadn't repaired her valve, this 40 yo woman  was 
going
to get a mechanical valve. Failed repairs present with recurrent  CHF.
Thrombosed mechanical valves or massive embolic strokes usually  
present a much
more
urgent picture.  As far as I can recall, I've never had  to emergently 
operate
a failed repair.
