Michael,
Though MR is very dependent on afterload I am not sure that this is the cause of her symptoms.( you can check this by an exercise tolerance test) For MR to be treated surgically:1.The patient must be symptomatic (i.e NYHA class III/IV) with severe MR
2.Asymptomatic but with severe MR and one or more of the following:AF,LA dilatation,LV dilatation,more than moderate increase in PAP
3.There is a need for cardiac surgery due to other pathologies and grade II or more MR is  present(Hal  has a wide experience in this field).
Regarding the aortic pathology I don't think she needs surgery at this moment.( as per my previous post)
