There are some cases of aortic stenosis, specially in females in whom aortic annulus is narrow and would not accept a valve size larger than 17 mm. As you know, we have 17 mm sized mechanical valves available from St Jude Medical and recently, from TTK. I recollect that my partners and I, starting from the year 2000, have implanted quite a few 17 mm sized valves in this type of cases with good short-term and long term outcome . In the case of the patient who received 18 mm ATS valve, she too was a short statured obese female, diabetic, with sedentary habit. To quote our teacher Dr Tendolkar's favourite comparison, she was "not going to participate in Asian Games like P. T. Usha did". So, I was happy to give her a valve that was a millimeter larger than the 17 mm St. Jude which I used to implant earlier.
I understand the application of aortic annulus widening procedures in these patients, so as to allow implantation of a valve that will prevent a mismatch.
Coming to the gradients, I sent her for echo just 3 or 4 days back during her follow up visit. I will retrieve the report from the echo department and will post it soon.
