Prasanna,
Do not placa a Bileaflet valve with the axis le3ttoright across the
orifice . This will leave onr leaqflet and orifice over the septal
bulge. The flow from this leaflet will be directed at the leser curve ,
may tend to open less and will be acompanied by less smooth flow into
the aorta and a higher gradient. t is preferable to have one end of the
hinge right over the septal area. Flow will then proceed evenly through
both orifices towards the greater curve and on around to the arch.

I've been using ATS mechanicals for about the last 3 years. No
problems so
far. The thing I like about it is the ability to reliably rotate it.
In
addition, I've had no problems with leaflet impingement which was a
problem with
the St. Jude Regent. For small calcified mitral annuli, they have an
AP valve
that sits on top of the valve orifice. This is a life saver if you are
faced
with an orifice smaller than 25 mm.

We implanted our units first ATS valve last week.(The so called
advanced
performance valve) Seemed OK and patient had low gradients etc (AVR)
Can members
who ave implanted it tell me about their experiences especially the
good the bad
and the ugly (More interested in the last two).
One thing I noted when we were implanting the valve was that we had
sized it to
a 24 AP (actually 26 sizer went in but 24 was the largest size
supplied) and it
did take a bit of effort to get it in (though once in it fit well). I
was a bit
worried at that time that it may have been "oversized" wrt to the
sizer.
Is there any 'preferred " position in literature wrt bilealfet valves
in the
aortic position. (I am aware of the position wrt monoleaflet valves but
could
not really get any data for bileaflet valves).
