I am not sure that I understand the message correctly when you say that St
Jude the original valve is easy to turn. Do you mean the rotatability of the
valve within the sewing ring? So that one can adjust the opening after
implantation. I remember very clearly that the original St Jude was not
rotateable and it only became so after Carbomedics was introduced. In fact
before Carbomedics another valve came into being but because of structural
problems was withdrawn the Duramedic Valve.

Actually I believe the original St Jude valve was and still is easy to turn,
but as they reduced the thickness of the cylinder through the new iterations
this feature became more theoretical than actual. Most of us in the US
probably use one of the newer designs. I can't keep up with the names but I
like the next to last version best as there is a little more give in the
sewing ring, housing complex that gives me a little more room for error in
implantation. I am willing to trade a tiny gain in EOA for a little ESA
(effective surgical area) not personally being an automaton nor the patient
symmetrical. For discussion I think there have been too many trade offs for
the theory (now dogma) of patient prosthesis mismatch (PPM). That is, I have
not seen that relief of PPM has restored patient survival to normal. I am
not, however suggesting that 19 size is good for everyone, but just that the
theory has not delivered the hype and we have traded for other problems.
Maybe Bob would like to either support or kill my impression if he can make
sense of it.

  For me, the ATS rotates more easily than the St Jude.
