[HSF] Anticoagulation for bioprosthetic AVR in NSR

Bioprosthetic aortic valves at our institution, in NSR, receive Enteric coated aspirin 81 mg daily for life. It has made things simpler for the pts and has worked. Seems like there was a survey published out of a presentation at WTSA a couple of years ago and that was the dominant cardiac surgical practice among respondents. chuckdouville

Same... at least for the first part. Unfortunately the first three months are also the most vulnerable to surgical bleeding for those that use for three months. I have definately lost patients in this period due to anticoagulation complications (tamponade).

We have not used any standard anticoagulation for bioprostheses alone for over 20 yrs. Anecdotally we have had no adverse effects.

va. History of polysubstance abuse, but denies iv drugs. Uneventful AVR with
Edwards bovine Magna valve, no annular abscess. (Ross was considered.)
neventful post op course, discharged in 5 days, chf resolved, nsr.
no anticoagulation
ASA only
Warfarin only--for how long
Clopidogrel
combination or other
