Don,
  I'm just referring to taking the quick, incomplete way out of a  complex 
problem in which revascularization should be only part of the  strategy.  I'm 
pleased to see that you include resynchronization therapy as  part of your 
treatment.  However, your approach fails to deal with the  usually significant MR 
and TR found in these often giant hearts.
