[HSF] Twisting of Vein grafts

Sylvia was checking my email and read me this topic. I said without looking that there would be many posts as it a small (but important) technical detail for which everyone will have a peculiar point that they wish to share.
Seems I was right for once. (Of course it is possibly for the wrong reason.)

If you do not have a flow probe , one way to check flows is to use a
cardioplegia line and run blood cardioplegia through it. The OM should
have a flow of 70 -100 ml / mi without the pump head pressure going
above 100 m Hg.(after dropping the heart ). Immediately tag the OM t the
aorta in the position you will suture it.(Or immediately anastomose it
on single clamp).
Also after coming of CPB you can use a hand held Doppler to check flows
and all grafts (Both IMA's and veins) should have a systolic and
diastolic component if flows are OK (ie whoosh-whoosh instead of whoosh)
per heart beat - subjective but useful if you have an ear for these things.
Marking with a pen or running a fine suture line on the adventitia will
also prevent rotation of grafts

We have checked the lie of the vein graft with heart in the lifted up position and thought that twist should have happened with changed lie of the graft after dropping it.
