It is pretty easy to deair while placing a patch on the anterior wall...

The original report was that of Jatene who did it open beating.
Incidentally while reading about air embolism, I remember reading
Lillehei's original technique of VSD closure with there being no cross
clamp. The principle was that as long as the hole in the RV was bigger
than the aorta, air would eject along the path of the larger hole which
was the basis by which they avoided air embolism. I know some surgeons
who repair the valve and place a large catheter (chest tube) across the
Mitral valve and just unclamp the aorta and close the LA while the heart
is beating and the heart automatically de-airs via the catheter (chest
tube) placed across the frustrated mitral valve which is in turn
connected to a vent.
