If the valve is mildly stenotic and mildly incompetent (and has three leaflets) it is likely to be rheumatic. Rheumatic aortic valves like this are very stable and if not hemodynamically significant can be followed for years. Note that bileaflet valves without important incompetence are also pretty stable.

Ajit,
If the AI is truly 1+ and tricuspid, I would definitely leave it. Long
term, the patient shouldn't need to be on Coumadin if you do a successful
mitral repair and maze.
