Great point Hal. We do take size into  consideration although we do not have an "index" per say. 
For true Marfans pts I would seriously start dialogue about surgery somewhere between 4 and 4.5cm. Bicuspids I would say 5 to 5.5cm.

Cleveland Clinic proposed indexing.  I think the size they  chose was 2.5cm/m2.   
 Also, what is the minimum size you recommend aortic  replacement for 
Marfan's or bicuspid aorta?
